Participants completing the survey may enter to win by providing their name and email address in the survey.
If you choose to provide your name and email address at the close of the survey you will become eligible to win an Apple iPad or equivalent prize. After the survey closes APICS will randomly select a name and email from names and email addresses provided in the survey. APICS will use the provided email to request information necessary to send the prize to the winner.
